A 72-year-old solid man presented with dyspnea and dysphagia following meals high in saturated fat. The investigation revealed that a mediastinal mass was responsible for the compression of the trachea and main carina. Fine needle aspiration guided by endobronchial ultrasound revealed a milky fluid with a high concentration of triglycerides and chylomicra. It was determined that a thoracic duct cyst was present. Due to the patient's refusal of more invasive therapeutic procedures, clinical stability was attained through conservative treatment.
